This form is for organisations that have experienced a data breach. If you're an individual wanting to report a breach of your personal information, please use our complaints tool.
Do not include any of the personal data involved in the breach in the form, such as the names of data subjects affected.
The information you provide must be accurate. You should give us as much detail as possible in this form. However, you can submit additional information afterwards by email.
Once you begin completing the form you will be unable to save it. Once you've completed the report, you will be able to review your answers. We'll then send you an email with a copy of your responses.
