This form is for organisations that have experienced a data breach. If you're an individual wanting to report a breach of your personal information, please use our complaints tool.

Do not include any of the personal data involved in the breach in the form, such as the names of data subjects affected. 

The information you provide must be accurate. You should give us as much detail as possible in this form. However, you can submit additional information afterwards by email.

Once you begin completing the form you will be unable to save it. Once you've completed the report, you will be able to review your answers. We'll then send you an email with a copy of your responses.

Why are you reporting the breach to the ICO? (optional)

If you want to make us aware of an incident that doesn’t meet the threshold to report, the quickest way to do this is to call our helpline on 0303 123 1113. You may not need to provide all the information requested in this form.

If you are unsure whether the incident meets the threshold to report, call our helpline on 0303 123 1113. It could save you time, as you may not need to report. We can also provide advice about steps to take following the breach.
